Canada's Drug Agency (CDA-AMC) has issued a positive final reimbursement recommendation for Loargys® (pegzilarginase) for eligible patients living with Arginase 1 Deficiency (ARG1-D).
This is an important step toward public access to a new treatment for Canadians living with ARG1-D, and an important example of community voices being represented in Canada's drug review process.
What is the CDA-AMC review?
After Health Canada authorizes a medication, Canada's Drug Agency evaluates clinical evidence, economic information and patient and clinician perspectives to make recommendations about whether participating public drug plans should reimburse the treatment.
During the Loargys review, CDA-AMC released a draft recommendation and invited feedback. CanPKU+ responded on behalf of the rare metabolic community, raising concerns about how treatment response would be measured and how the proposed criteria could affect people living with ARG1-D.
Community advocacy made an impact
The final recommendation reflects several important issues raised through our advocacy.
- Stability matters. In a progressive disease such as ARG1-D, preventing further decline can be a meaningful treatment outcome. The final guidance recognizes stabilization alongside improvements in areas such as mobility, communication, quality of life and participation in everyday life.
- One blood test does not tell the whole story. The final guidance recognizes that a temporary increase in arginine alone should not necessarily be considered evidence that treatment is ineffective when the person's overall clinical picture remains stable.
- Meaningful change can take time. The recommendation acknowledges that changes in mobility, cognition and development may take 12 to 24 months or longer to become apparent.

Our community's words are reflected in the final recommendation. Language submitted by CanPKU+ about what meaningful treatment benefit looks like, including preserving mobility, maintaining communication and participating in school and community life, appears in the final CDA-AMC implementation guidance. Seeing the community's priorities reflected so directly in the final document is a powerful example of why patient and caregiver voices belong in reimbursement decisions.
What happens next?
A positive CDA-AMC recommendation is an important milestone, but it does not mean Loargys is automatically publicly funded across Canada. The reimbursement process now moves forward through additional negotiations and decisions by participating federal, provincial and territorial drug plans.
CanPKU+ will continue to follow this process and advocate for fair, consistent and timely access for Canadians living with ARG1-D.
